All you need to know about billing and coverage
You should present the following information and documents to the Scheduling Office.
The administrator will give you an admission form for hospitalization and will get in contact with your insurance company in order to get a coverage approval for your operation, or in some instances will get the insurance approval on your own.
You will then need to check with the hospital administration 3 days before your scheduled operation day to see if they have received your insurance approval.
Once your coverage insurance is approved, you should be present at the hospital 48 Hours prior to your operation day and submit to the Admission Office on level -1 the following documents:

You should present the following information and documents to the Scheduling Office.
The administrator will give you an admission form for hospitalization and a NSSF form that you need to present to the NSSF center in order to get your coverage approval from the NSSF.
Once you get your NSSF coverage approval, you should be present at the hospital 48 Hours prior to your operation and submit to the Admission Office on level -1 the following documents:

You should present the following information and documents to the Scheduling Office.
The administrator will give you an admission form for hospitalization, a COOP form along with a medical prescription.
Then you need to present all these documents to the COOP center in order to get an approval for undergoing your operation.
Once you get your COOP coverage approval, you should be present at the hospital 48 Hours prior to your operation day and submit to the Admission Office on level -1 the following documents:

The administrator will give you an admission form for hospitalization, a report confirming that you have been transferred from the Army Hospital to the Eye & Ear University Hospital and a Proforma if needed (depending on the type of operation)
Then you need to present all these documents to the Army center in order to get an approval for your operation( بطاقة تعهد و أخذ على العاتق)
You should be present at the hospital 48 Hours prior to your and submit to the Admission Office on level -1 the following documents:
